Please fill out one sheet representing the most frequently consumed foods. Include everything that you eat and drink and how much. This will allow a proper nutritional analysis and reasonable suggestions for change.
Food Eaten (Amount)Times Eaten ( approx. 4oz = 1 can tuna, 8oz = 2 cans of tuna )
Example: 4 oz oatmeal (separate each meal apart)
4 oz apples sliced with 1 tbsp chopped walnuts
4 oz 2% milk (show approx. portions)
